There are three sections on this form: Picture links, payment and order form. Please complete the steps in this order.
1) VIEW YOUR PHOTOS - Click on your team's photos using the link below. You do not order anything directly from Image Arts - the photos are for viewing only. Find the photo you wish to use and note the number listed below the photo. You will input that photo # on this order form. At the very bottom of the screen, click NEXT to advance to the next section of the form.
2) PAY FOR YOUR BUTTONS - Complete the payment link for the amount of buttons you wish to order. Buttons are $10 each with a maximum order of 10 buttons per order form. If you'd like to order more than 10 buttons, please complete a second order form. Please note your 13 digit Transaction Payment Number from your online payment receipt, as you will be required to enter that on this order form. At the bottom of the screen, click NEXT.
3) COMPLETE THIS BUTTON ORDER FORM - Please be sure to click the SUBMIT button at the end of the form. Your order is not complete until it has been submitted to us and you receive an email confirmation of your order. Thank you!
If you prefer to pay via check, you can drop off a check at the high school, attention Karen Svetlay, payable to MBHS Dorians. Please note player's name and "Brag Button" on the check. We do not accept cash. Buttons will not be made until payment is received.
If you need additional buttons, you must submit a new order form. We are ALWAYS accepting orders and making buttons!
